How to do it
Sitting tall, slowly rotate your head to the right, then to the left. Next flex your chin toward your chest, then look up toward the ceiling. Finally, tip your ear toward each shoulder. Move only as far as is comfortable.
What to watch for
Note which directions are stiff, painful, or clearly more limited than the opposite side. Compare left and right rotation, and how far your chin can tuck versus how far you can extend backward.
What a positive test suggests
Painful or restricted ranges point to where the dysfunction sits. Limited rotation often reflects upper cervical joint restriction, while limited extension frequently points to lower cervical or facet joint involvement.
How to do it
Sitting relaxed with your shoulders down, place your hands under your chin and the base of your skull. Gently lift your own head upward, creating a small amount of traction through the neck. Hold for 20 to 30 seconds.
What to watch for
Notice whether your neck pain, arm pain, or tingling decreases while you hold the lift. Many patients describe an immediate sense of relief or reduced pressure.
What a positive test suggests
Symptom relief with gentle traction suggests nerve root or disc involvement. Decompression of the cervical joints reduces pressure on inflamed nerve roots, and this is a strong positive sign for responsive conservative care.
How to do it
Using your fingertips, press firmly along the ridge of your upper trapezius, then up under the base of your skull (suboccipitals), and finally at the inside corner of your shoulder blade (levator scapulae). Compare each side.
What to watch for
Tender spots, ropey bands, or knots that feel markedly tighter on one side than the other. Some spots may refer pain upward into the head or outward toward the shoulder blade.
